Measure 1 – Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9%)

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 1 requires documentation of a HbA1c test performed during the measurement period with a value of greater than 9%.

 

NOTE: This is an inverse measure. Report the performance met code (3046F or 3046F with 8P) when HbA1c levels for a patient exceed 9%. The performance not met code (3044F or 3045F) indicates better quality care.

This documentation may be found in the patient’s medical record and must have been performed by the reporting physician. The relevant document should be made available to the auditor for the patient requested.
