Measure 130 – Documentation of Current Medications in the Medical Record

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 130 requires documentation of a patients’ current medications using all immediate resources available on the date of the encounter.

  • NOTE: The list of current medications must include ALL known prescriptions, over-the-counter drugs, herbals, and vitamin/mineral/dietary (nutritional) supplements AND must contain the medications’ name, dosage, frequency and route of administration.

This documentation may be found in the patient’s medical record. The relevant document should be made available to the auditor for the patient requested.
