Measure 223 – Functional Status Change for Patients with General Orthopaedic Impairments

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 223 requires documentation that the Focus on Therapeutic Outcomes (FOTO) General Orthopaedic Functional Status Patient Reported Outcome Measure was administered at admission (intake) and discharge and that the patient’s Risk-Adjusted Functional Status Change Residual Score is recorded in the medical record. This measure is to be performed for all patients aged 14 years and older with general orthopaedic impairments who have initiated rehabilitation treatment.


This documentation may be found in the patient’s medical record. The relevant document should be made available to the auditor for the patient requested.


What tools may be used for this measure?

This measure requires the use of the proprietary Focus on Therapeutic Outcomes (FOTO) General Orthopaedic Functional Status survey instrument. Physician practices are given a limited license for use of the paper version of the survey for purposes of MIPS reporting, links to these surveys are available in AAOE Empower.
