Measure 24 – Communication with the Physician or Other Clinician Managing Ongoing Care Post-Fracture for Men and Women Aged 50 Years and Older

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 24 requires documentation of communication from the clinician treating a patient fracture to the clinician managing the patient’s on-going care. Documentation must state that the treating clinician notified the managing clinician that the fracture occurred, and that the patient was or should be tested or treated for osteoporosis.

This documentation may be found in the patient’s medical record. The relevant document should be made available to the auditor for the patient requested.
