Measure 355 – Unplanned Reoperation within the 30-Day Postoperative Period

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 355 requires documentation that a patient had an unplanned reoperation with the 30-day postoperative period.


NOTE: This is an inverse measure, the lower the percentage of patients undergoing an unplanned reoperation, the better quality of care provided.


This documentation may be found in the patient’s medical record or operative notes. The relevant document should be made available to the auditor for the patient requested.
