Measure 402 – Tobacco Use and Help with Quitting Among Adolescents

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 402 requires documentation that the eligible clinician documented patients aged 12-20 years old tobacco use status and provided help with quitting if identified as a tobacco user.

 

This documentation may be found in the patient’s medical record. The relevant document should be made available to the auditor for the patient requested.

 

What qualifies as an intervention for this measure?

An intervention for this measure may be documented as:

  • Advice given to quit smoking or tobacco use,
  • Counseling on the benefits of quitting smoking or tobacco use,
  • Assistance with or referral to external smoking or tobacco cessation support programs,
  • Current enrollment in smoking or tobacco use cessation program.

For purposes of this measure, one of the above interventions must be documented in the patient’s medical record.
