Measure 412 – Documentation of Signed Opioid Treatment Agreement

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 412 requires documentation of a signed opioid treatment agreement for patients prescribed opiates for longer than six weeks.


This documentation may be found in the patient’s medical record. The relevant document should be made available to the auditor for the patient requested.


What should the agreement include?

For this measure, your opioid treatment agreement should include:

  • Potential risks of continuous opioid treatment
  • Alternatives to continuous opioid treatment
