Measure 46 – Medication Reconciliation Post Discharge

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 46 requires documentation in the outpatient medical record that medication reconciliation (review of discharge medication reconciled with the most recent medication list in the outpatient record) and must include evidence of medication reconciliation and the date on which it was performed. To qualify for this measure, the patient must have been discharged from any inpatient facility within 30-days prior to the documented medication reconciliation. Note the date of the patient’s discharge in the medical record.

This documentation may be found in the patient’s medical record. The relevant document should be made available to the auditor for the patient requested.
