Measure 474 – Zoster (Shingles) Vaccination

Audit Criteria

When we’re reviewing your clinicians’ data, it may be helpful to know what we are looking for in order for us to verify to CMS that your data is accurate and complete. Measure 474 requires documentation of a patient’s (age 50 or older) shingles vaccination status in the medical record.

 

This documentation may be found in the patient’s medical record. The relevant document should be made available to the auditor for the patient requested.

 

Who can report this measure?

This measure may be reported by any eligible clinician. Reporting clinicians do not have to administer the Shingrix vaccination however, they must document that the vaccination was previously administered.
